Description
A white card (4″ x 6″) to record the student’s name, address, age, grade, birth date, and date of salvation. This information is to be filled out when you receive the answer sheet from their first lesson. This is the same as the “CHILD CARD” but all teen, young adult and adult courses are listed to record the date and grade by the correct lesson and course.
